What am I going to do when I’m not receiving treatment? What is mom going to do here all day? These are important questions. Our activity director designs programs to enrich your physical, mental, social, and spiritual well-being here at Azalea Heights. You’ll experience daily and monthly activities, including individual and group activities to create a sense of community. By building relationships and encouraging participation, your care team helps you to continue being part of a community. Activities include art, music, gardening, entertainment, and outings.

  Sometimes after a hospital stay, your doctor will recommend you continue your care at a skilled nursing community before you go home. You have the option to choose Touchstone for your skilled nursing care.

  We understand that not everyone needs to stay with us long term. Our skilled nursing care programs are designed specifically to help you get the care you need to get you back to doing the things you love.

  Part of your skilled nursing care plan might include rehabilitation, which you can learn more about in our rehabilitation section. So, whether you need rehabilitation after undergoing knee surgery, or your loved one had a stroke and needs speech therapy, our skilled nursing care programs can help.

  We know that transitioning to long-term care is a difficult journey for you and your loved one. We will do everything we can to make you feel at home. Your team will encourage your independence with healthy activities, and give around-the-clock nursing support.

  Long-term care can be appropriate when:

  Moving around safely in the home isn’t possible. Frequent falls are becoming a concern. Regular bathing and adequate hygiene are not being maintained. There’s increasing dependence on help with eating, dressing, and bathing. Outside help is needed for housekeeping and meal preparation.

  There’s a loss in weight, dehydration, refusal to eat, or inability to eat properly. There’s an inability to take medications on schedule and at the correct dosages. Home care assistance is not an affordable option.

  You’re thinking about coming to a Touchstone Community, but you’re worried about how you’ll get to your dialysis appointments or doctor visits. Or maybe you need to admit to a community at 2 a.m. on a Sunday night. We’ve got you covered. With support services, we will drive to your doctor appointments, offer dialysis transportation, accept admissions and offer care 24/7, design a diet for you with our registered dietitian, help with discharge planning, and much more. We take responsibility for your overall well-being, so if that means you need additional help outside of a Touchstone community, we will still find a way to support and help you.
