One year ago today, they launched TOUCHSTONE HEALTH with partner Heritage Management based in Albuquerque, New Mexico. The new partnership, TOUCHSTONE HEALTH provides home health care and hospice with an approach that is unique to San Antonio.
TOUCHSTONE COMMUNITIES’ partner Heritage Management has long been known for their innovative approach to health care. For over a hundred years, Heritage Management has worked to create best in class health care experiences for the patients and families that they serve. The partnership brings a unique transitional care model that no other company provides in San Antonio. One of the advantages of this model allows a patient to be evaluated by a nurse while still in the hospital or skilled nursing facility to get a head start on planning their transition to the next level of care. The transitional care nurse coordinates care with all parties involved in managing the patient’s care. This approach allows TOUCHSTONE HEALTH to support a seamless experience for patients across the care continuum wherever they are going, even when this provider is not Touchstone.
TOUCHSTONE HEALTH has been providing home health since they opened exactly one year ago today. The company is excited to announce they have received their CHAP certification for hospice services. This certification has been awarded to the company after an extensive survey process in which they received zero deficiencies, a significant accomplishment. TOUCHSTONE HEALTH will be providing hospice and palliative care services to San Antonio in addition to home health.
