Return-To-Home™
In addition to providing our residents with expert rehabilitative services, the Touchstone Communities offer services for patients needing short-stay rehabilitation, to help them recuperate between hospital stays and going home. Our special Return-to-Home™ Program, which has an expert staff that works keep patient stays from 30 to 45 days or less, specializes in stroke, hip/knee replacement and other complex medical conditions. Individuals in our Return-to-Home™ program get the specialized care they need with the focus on getting well and returning home as soon as possible.
