TOUCHSTONE'S NEW "NEIGHBORHOOD CONCEPT" HOME
TO BE BUILT IN ATASCOSA COUNTY
Touchstone Communities’ planned facility, The Heights, will be the site of the first rehabilitation and long term care community in Texas to provide a more intimate level of care for residents. Known as the “neighborhood concept,” Touchstone will utilize this nationally emerging design for creation and operation of our new site. Residents will also benefit from the expertise of a Touchstone staff that has built a reputation for exceptional care of families and mentoring employees across its thirteen communities in Texas and Arizona.

FEATURES:
- The Heights will be built in Atascosa County and will serve nearby communities such as Pleasanton, Jourdanton, Poteet, Lytle, Charlotte and Christine. It opens the summer of 2008.
- Approximately 100 employees will be hired from the surrounding area.
- Availability of private and semi-private rooms for up to 100 residents.
- “Neighborhood” concept – smaller, more intimate units staffed with self-directed teams of highly trained caregivers. Food is available to residents at all times; caregivers are empowered to listen and respond to resident needs.
- Beautifully landscaped public spaces and walkways.
INNOVATIVE “NEIGHBORHOOD CONCEPT”
- Neighborhoods are clusters of private and semi-private rooms for about 33 residents. Each neighborhood has its own staff of professionals who offer 24-hour care in a home-like atmosphere.
- Our neighborhood interior design has been customized with a warm and inviting color palette and appealing, high quality fixtures and furnishings.
- Much like with their own homes, residents will benefit from a focus on the neighborhood’s dining room and family area, complete with fireplace and television, which serves as the central gathering spot for meals and activities. Different activities will be conducted simultaneously in each of the three neighborhoods, allowing residents to choose activities they enjoy most.
- The goal of our expert care team within each neighborhood is to respond to each resident's needs and personal choices. Every staff member is there to honor the dignity and preferences of residents, and to make every effort to encourage and help restore wellness. Touchstone Communities sets standards at nationally recognized levels for resident-directed care.
HIGHEST LEVEL OF STANDARDS FOR REHABILITATIVE AND LONG TERM CARE WILL BE PROVIDED AT THE HEIGHTS
Touchstone's care team works to promote independence for residents. Patients and their families will appreciate:
- Receiving the same caliber of rehabilitative care offered in specialty hospitals in larger cities;
- A Return to Home™ program focused on helping patients attain or surpass their prior level of functioning;
- All residents are under the direct supervision of a physician;
- Residents enjoy around-the-clock nursing care supervised by their physician and a physician is always on call;
- Individualized programs designed to meet specific needs and preferences;
- Each member of Touchstone’s team of occupational, speech and physical therapists is licensed by the State of Texas
